Auspicing Agreement

Below is an online form for filling out. If you are already a member of UPM, you will not see this form when logged in. The workaround is for you to use a PDF form. Please let us know if you want one.

As part of the form process, we require a signature from you. Please download the following form with an agreements, sign it and return it to secretary@upm.org.au.

After you have completed the form, we will make an assessment at our next general meeting and get back to you about your application. Alternatively, you can fill out the PDF version of the form available from the link below.
